The present study aimed to examine the effectiveness of logotherapy on loneliness and fear of death in older adults with chronic physical illnesses. This study was a quasi-experimental design with a pretest–posttest structure, including a follow-up phase and a control group. The statistical population consisted of older adults with chronic physical illnesses referring to specialized clinics in Tehran in 2025, from whom 30 participants were selected through purposive sampling and then randomly assigned to experimental and control groups (15 participants in each group). The experimental group received logotherapy in eight 90-minute sessions, while the control group did not receive any intervention. The research instruments included a Loneliness Questionnaire and a Fear of Death Scale. Data were analyzed using repeated measures analysis of variance. The results indicated that logotherapy significantly reduced loneliness and fear of death in the experimental group, and this effect remained largely stable at the follow-up stage. Accordingly, logotherapy can be considered an effective psychological intervention for reducing loneliness and fear of death and improving the mental health of older adults with chronic physical illnesses.
